Payment Information

The total fee for this service is a rate of_______  / hour. (    pm-        am ) for  hours of Entertainment.  $ total cost $ . 
50% must be paid to secure the date of the event in which you can mail or email money transfer it.

This deposit is non refundable as it secures the date in which services are to be required. 

The other 50% balance will be required before the start of the event on the day of. 

If ( you) requests additional hours, and ( AMM Music Productions ) is able to provide them, the fee will be ( $$ dollars) per additional hour. 

If ( you ) wishes to cancel the services, he/she must give ( AMM Music Productions ) at least 14 days notice. Any cancellation after that point will result in ( forfeiture of deposit, balance due immediately,)



Setup


( AMM Music Productions ) will bring the following for setup: (List of what Dj will be bringing). 

Mixer, PA system, computer, lights, Mics, and proper cords. 

( AMM Music Productions) will require venue to provide list of what the customer or venue will provide. 

Such as stage or area to set up at, internet, tables and 2 chairs to set up on, and proper electrical plugs for equipment.
